<p>At first, your newborn will likely fall asleep whenever and wherever he wants. &ldquo;His governing factors are sleep and hunger, and they override everything else,&rdquo; says Jodi Mindell, Ph.D., author of <em>Sleeping Through the Night </em>(2005). You won&rsquo;t get him on a nap schedule yet, but you should make sure he wakes for feedings every few hours during the day. When he&rsquo;s 3 months old, you can start to get <a href="http://www.fitpregnancy.com/yournewlife/motherhood/sleep-strategies-for-both-of-you-40726707.html">serious about snoozing</a>. After all, sleeping regularly makes for a less cranky baby&mdash;and mom, Mindell says. Here&rsquo;s her advice:</p>
<p>* <strong>Avoid weekend sleep-ins</strong>:&nbsp; Wake your baby around the same time every morning. Try to adhere to set nap times: 9:30 a.m., 2:30 p.m., and possibly 5:30 p.m., if he seems drowsy. If that&rsquo;s too rigid for you, put your child to bed two hours after he wakes up, whenever that is throughout the day.</p>
<p>* <strong>Set the stage for success</strong>:&nbsp; Signal it&rsquo;s time to sleep with a shortened version of your nighttime routine. &ldquo;If you typically sing lullabies to your baby at night, sing a few before his nap as well,&rdquo; says Mindell.</p>
<p>* <strong>Know what&rsquo;s typical:&nbsp;</strong> By about 9 to 10 months of age, most babies should rest twice a day&mdash;at around 9 a.m. and 2 p.m.</p>
<p>* <strong>Don&rsquo;t sub the car seat for the bed</strong>:&nbsp; As often as possible, aim to have your baby nap in the same spot where he sleeps at night. And if you get in the habit of nursing him to sleep or putting him in a stroller or swing, be forewarned: Eventually he won&rsquo;t be able to fall asleep any other way.</p>

<p>If you&rsquo;ve had a Cesarean section, you may have trouble getting a second one covered by insurance if you apply for an individual policy (one that you purchase on your own). Once you&rsquo;ve had a C-section, the chances that you&rsquo;ll have another are about 90 percent, according to the <a href="http://www.cdc.gov/nchs/">National Center for Health Statistics</a>. And since C-sections cost about 76 percent more than vaginal deliveries, some individual insurers are rejecting women who&rsquo;ve had the operation&mdash;or charging them higher premiums or deductibles.<br />
<br />
Your best defense: Shop around for a policy that doesn&rsquo;t discriminate&mdash;or find out whether you can get coverage through your state. If you&rsquo;re rejected or discover that your premium is being raised, check with your state insurance commissioner to see whether you can fight back (some states prohibit discrimination based on medical history). If your state doesn&rsquo;t regulate this practice, consider contacting your state legislators. <br />

<p>You know your delivery probably won&rsquo;t go down like it did in <em>Knocked Up</em> or on <em>ER </em>(let&rsquo;s hope), but what labor and childbirth look like has remained mostly a mystery to modern women&mdash;until now.</p>
<p>Thanks to YouTube, a curious mom-to-be can view the nitty-gritty details of thousands of deliveries, mainly home births. A Virginia natural childbirth organization in Virginia even held a contest for the best birth video. While experts don&rsquo;t recommend YouTube as a primary source of birthing information, &ldquo;many of the videos offer an honest, woman-to-woman perspective of the experience,&rdquo; says Donyale Abe of the International Childbirth Educators Association. &ldquo;You rarely get that elsewhere.&rdquo;</p>

								<p><strong>Fear of sex repercussions</strong>: It&rsquo;s debatable how many Cesareans are performed purely because a woman requests one, to make sure her doctor, rather than the doctor on call, delivers her baby or because she has fears about labor or vaginal delivery. But doctors say some women opt for a C-section because they worry that a vaginal birth will impair their capacity for sexual pleasure. &ldquo;They fear that their vagina is not going to be as tight,&rdquo; says OB-GYN Bonnie Wise, M.D. &ldquo;But the ability to orgasm has nothing to do with how tight the vagina is. It has to do with stimulating the clitoris.&rdquo;<br />
<br />
<strong>Fear of incontinence</strong>: While it&rsquo;s true that in the first months postpartum women who deliver vaginally have a relatively high rate of urinary incontinence&mdash;about 16 to 20 percent&mdash;the effects typically aren&rsquo;t lasting or severe. Even women who deliver via C-section have an initial incontinence rate of about 10 percent, simply as a result of having been pregnant. &ldquo;The weight of the pregnancy for all those months can break down the muscle fibers in the pelvic floor,&rdquo; Wise explains. Performing Kegel exercises during pregnancy can help prevent incontinence.</p>

								<p>If nausea, heartburn or other annoying symptoms are plaguing your pregnancy, you might turn first to natural remedies, assuming they&rsquo;re safer than prescription or even over-the-counter options. But that&rsquo;s not necessarily true. Certain herbs, for example, can cause uterine contractions that could lead to preterm labor.</p>
<p>You also shouldn&rsquo;t be in a rush to self-medicate. &ldquo;Have your symptoms evaluated by an obstetrician before you try to treat them yourself, to make sure what you&rsquo;re experiencing isn&rsquo;t a sign of something serious,&rdquo; advises OBGYN Joel M. Evans, M.D., founder of <a href="http://www.centerforwomenshealth.com/" target="_blank">The Center for Women&rsquo;s Health</a> in Stamford, Conn., and author of <em>The Whole Pregnancy Handbook</em> (2005).</p>
<p>Next, explore your options. As with all medicines, &ldquo;It&rsquo;s best to start with food and lifestyle changes, and use herbs and other remedies last,&rdquo; says Morgan Martin, N.D., L.M., chairwoman of the Naturopathic Midwifery Department at Bastyr University in Kenmore, Wash. For example, exer cise can do wonders for constipation and insomnia. Massage and acupuncture can also ease many ills, but only consult licensed practitioners who have experience treating pregnant women.</p>
<p>The following remedies, available at natural foods stores or online, are all safe and effective during pregnancy; regardless, always tell your doctor or midwife what you&rsquo;re taking. You&rsquo;ll also find remedies that might ordinarily be safe but should be avoided for now. And, of course, exercise extra caution in your first trimester.</p>
<p><strong>Morning Sickness</strong></p>
<p>&bull; Eat small meals and snacks throughout the day to keep food in your stomach.<br />
&bull; Wear acupressure wristbands (such as Psi Bands), whch stimulate acupressure points.<br />
&bull; Prenatal vitamins can exacerbate nausea; take yours with food or at night so you can sleep through the side effects. Or try prescription liquid vitamins instead.<br />
&bull; Take powdered ginger capsules (250 milligrams up to three times daily); drink 4 ounces of fresh ginger tea (peel, slice thin and simmer in 8 ounces of hot water) two to three times a day; mix 5 milliliters of ginger tincture with enough water to fill a 1-ounce bottle; take five to 10 drops up to six or seven times a day; or suck on candied ginger or drink brewed ginger ale.<br />
&bull; Drink up to four 8-ounce cups a day of chamomile, peppermint or raspberry leaf tea.</p>
<p>Use tea bags or steep 2 teaspoons of the herb in 8 ounces of hot water for two to three minutes, then strain.</p>
<p><strong>Avoid</strong>: Herbs that contain volatile (essential) oils can be dangerous during pregnancy; use them sparingly and topically only. In general, only use herbs in a dilute form, such as tea.</p>

<p><strong>Heartburn</strong></p>
<p>&bull; Avoid spicy or acidic foods.<br />
&bull; Eat small meals frequently and slowly throughout the day.<br />
&bull; Chew a parsley sprig after eating.<br />
&bull; Chew fennel seeds or steep 4 tablespoons of the seeds in 1 quart of water, then strain.<br />
&bull; Suck on slippery elm bark lozenges or mix the powdered herb with water, juice or milk. To make tea, use bags or pour 8 ounces of hot water over 1 teaspoon of slippery elm powder. Drink up to four 8-ounces cups a day.<br />
&bull; Take two deglycerated licorice root capsules after each meal and two before bedtime.</p>
<p><strong>Avoid</strong>: Don&rsquo;t use slippery elm powder for more than 14 consecutive days. Don&rsquo;t take medications like Zantac without consulting your doctor.</p>
<p><strong>Constipation</strong></p>
<p>&bull; Get 25-30 grams of fiber daily from fruits, vegetables, legumes (beans and peas) and whole grains.<br />
&bull; Drink up to 8 ounces of water or clear fluids (like herbal teas or broth) every hour.<br />
&bull; Sprinkle ground flaxseed on cereal, salads, etc.<br />
&bull; Take probiotics.<br />
&bull; Try psyllium powder, such as Metamucil, or flaxseed.</p>
<p><strong>Avoid</strong>: Mineral oil or herbs that cause a laxative effect by irritating the colon (such as senna or cascara sagrada); both can stimulate uterine contractions.</p>
<p><strong>Swelling/Bloating</strong></p>
<p>&bull; Sit when you can with your feet elevated and gently massage your legs. Drape a cloth saturated with chilled witch hazel over your feet.<br />
&bull; Wear support hose if you stand a lot.<br />
&bull; Stay hydrated.<br />
&bull; Avoid processed foods containing a lot of sodium.<br />
&bull;&nbsp; Steep &frac12; teaspoon each of parsley leaf and dandelion leaf in 8 ounces of boiling water for five minutes and drink once or twice a day.<br />
&bull; Eat raw cucumbers or dandelion greens or saut&eacute; the greens with garlic and olive oil; all are gentle diuretics.</p>
<p><strong>Avoid</strong>: Over-the-counter diuretics.</p>
<p><strong>Insomnia</strong></p>
<p>&bull; Eat a late-night snack with protein to keep blood sugar levels steady throughout the night.<br />
&bull; Sleep with pillows under your abdomen or between your legs.<br />
&bull; Sprinkle lavender and rose petals or essential oils into a warm bath before bed, or keep something lavenderscented by your bed.<br />
&bull; Drink relaxing herbal teas like Sleepytime, or make a tea from linden flower, rose petals, chamomile, lemon balm and lavender (mix 1 tablespoon of each in a container, steep 1 teaspoon of mix in boiling water).</p>
<p><strong>Avoid</strong>: Prescription or OTC sedatives; check with your doctor or midwife before using valerian.</p>

							How chewing gum now can benefit your baby

								<p>The condition of a mom-to-be&rsquo;s teeth and gums can influence her pregnancy&mdash;and her baby&rsquo;s dental health. Researchers at New York University found that women with gum disease are at higher risk for gestational diabetes, and a small Chilean study recently discovered gum-disease bacteria in the amniotic fluid of women threatened with premature labor. </p>
<p>Meanwhile, the biggest predictor of cavities in 3-year-olds is whether a child was exposed to plaque-producing bacteria before age 1; moms are the most common source of exposure, through sharing spoons, for example. In the month before your due date, reduce mouth bacteria by chewing gum with at least 67 percent xylitol (such as Spry or Xylichew) twice daily for one minute after a meal. See your dentist regularly and floss three times daily throughout pregnancy to keep bacteria at bay.</p>

								<p>Celebrities don&rsquo;t just help motherhood en vogue; they also focus attention on causes important to families, such as efforts to end poverty, hunger and childhood disease. And while many of us are familiar with actress Angelina Jolie&rsquo;s good deeds on behalf of the <a href="http://www.unhcr.org/4ac61fae9.html" target="_blank">United Nations</a>, and former model Christy Turlington Burns&rsquo; work for <a href="http://www.fitpregnancy.com/blog/labornurse/christy-turlington-and-care-40480577.html">CARE</a>, we thought you should know about some other stars&rsquo; charitable efforts.<br />
<br />
<strong>Natalie Imbruglia: </strong><strong>ending obstetric damage</strong> As spokeswoman for the <a href="http://www.endfistula.org/" target="_blank">Campaign to End Fistula</a>, singer and actress Natalie Imbruglia has visited Nigeria several times to raise awareness about a devastating condition in which prolonged labor without medical attention causes a hole to develop in the birth canal. Unable to control their feces and urine, women with obstetric fistula are often shunned by others. Extremely rare in developed countries, the condition affects between 50,000 and 100,000 women each year in developing nations. &ldquo;Fistula is difficult to talk about,&rdquo; says Imbruglia. &ldquo;But that discomfort pales in comparison to what women living with it face every day.&rdquo; Since the campaign was launched in 2003, about 7,800 affected women have been treated, and progress has also been made in the area of prevention.<br />
<br />
<strong>Lucy Liu:</strong> <strong>preventing child deaths</strong> A longtime UNICEF ambassador, actress Lucy Liu was recently honored by the organization for her &ldquo;tireless work on behalf of the world&rsquo;s most vulnerable children.&rdquo; She promotes and supports the <a href="http://www.unicefusa.org/campaigns/believe-in-zero/" target="_blank">Believe in Zero campaign</a>, which aims to put an end to the deaths of 25,000 children each day from preventable causes such as malnutrition, disease and contaminated water. Liu continues to travel widely for UNICEF, blogging and logging video reports from the Ivory Coast, Russia and the Congo.<br />
<br />
<strong>Salma Hayek:</strong> <strong>taking on tetanus</strong> She may have raised eyebrows earlier this year for nursing another woman&rsquo;s hungry baby during a goodwill trip to Sierra Leone, but actress Salma Hayek was more than happy to make a point by sharing her milk. Because of cultural taboos, many women in this African country breastfeed for only a few months. Partly as a result, the infant mortality rate is the highest in the world. Hayek, mother of 1 1∕2-year-old Valentina, toured Sierra Leone with ABC&rsquo;s Nightline to call attention to tetanus, another contributor to infants&rsquo;&mdash;and mothers&rsquo;&mdash;deaths in developing countries. Since 2008, Hayek has served as spokeswoman for the <a href="http://www.unicef.org/southafrica/support_4723.html" target="_blank">Pampers One Pack = One Vaccine initiative</a> to help support UNICEF&rsquo;s mission to eradicate tetanus by 2012.<br />
<br />
<strong>Amanda Peet: advocating for vaccination</strong> Motivated by her struggle to separate fact from fiction when deciding whether to vaccinate her daughter, Frankie, now 2, actress Amanda Peet joined <a href="http://www.ecbt.org/" target="_blank">Every Child by Two</a> and the <a href="http://www.aap.org/" target="_blank">American Academy of Pediatrics </a>last year to encourage parents to have their children vaccinated on schedule. &ldquo;There is a wealth of misinformation about vaccines out there, particularly in Hollywood,&rdquo; Peet says. &ldquo;But talking with doctors and reading studies made it clear to us that delaying vaccinations could jeopardize our baby&rsquo;s life.&rdquo; As a volunteer spokeswoman for the Vaccinate Your Baby campaign, Peet&rsquo;s messages have reached 500 million people in less than a year.</p>

							Modern moms find pregnancy no reason to slow down

								<p>Maybe it&rsquo;s a sign of our multitasking times: Today&rsquo;s expectant mothers remain on the go&mdash;exercising, performing physical work at home or on the job and having sex&mdash; almost until they give birth, researchers have found.<br />
<br />
It&rsquo;s been assumed that moms-to-be take it easier in the last three months. But a survey of 425 women found that 60 percent reported strenuous physical activity, including exercise, during the third trimester, and 62 percent reported having sex. In the two weeks before delivery, 49 percent said they&rsquo;d exercised and 40 percent said they&rsquo;d had sex. Even in the 48 hours before giving birth, most spent at least five hours on their feet per day; one third said they&rsquo;d worked out and 17 percent had sex. (Some may have had intercourse because they believed it brings on labor, though a recent study found it doesn&rsquo;t.) Doctors say there&rsquo;s no obligation to slow down if you feel good. &ldquo;Each pregnancy is different, as is each pregnant woman,&rdquo; says study author Nathan Fox, M.D. &ldquo;Unless there&rsquo;s a reason not to, pregnant women should be encouraged to remain active during pregnancy.&rdquo;&nbsp;<br />

							Pregnant women are at very high risk from the H1N1 (swine flu) virus. Here are expert answers to questions about the vaccine.

								<p>Public health experts continue to warn that the 2009 H1N1 virus (aka swine flu) has the potential to cause widespread, serious illness&mdash;even death. And because pregnant women are among those at highest risk, officials are recommending that they be at the front of the line to receive the vaccine. Clinical trials on pregnant women have been conducted, and a vaccine became available in October.</p>
<p>The introduction of any new vaccine can raise concerns, especially when pregnancy is involved. To help address them, here are answers to some of the most common questions about this strain of the influenza virus. For more information, including breaking news and updates, visit <a target="_blank" href="http://www.flu.gov">flu.gov</a>.</p>
<p><strong>Is the 2009 H1N1 vaccine recommended for all pregnant women?</strong></p>
<p>Yes&mdash;except for women who cannot receive the regular seasonal flu vaccine because they are allergic to eggs. (Both vaccines contain a virus that is grown in eggs.) &ldquo;We&rsquo;re recommending that all other pregnant women get the vaccine as soon as it&rsquo;s available, regardless of their stage of pregnancy,&rdquo; says Anthony S. Fauci, M.D., director of the <a href="http://www3.niaid.nih.gov/" target="_blank">National Institute of Allergy and Infectious Diseases</a> at the National Institutes of Health. Expectant women should not receive the nasal spray flu vaccine.</p>
<p><strong>Why is it so important that pregnant women be vaccinated?</strong></p>
<p>Expectant women are more susceptible to serious complications from infection with all types of influenza viruses, including the 2009 H1N1. &ldquo;There is not only an increased risk of being hospitalized, but we&rsquo;ve found a disproportionate number of deaths among pregnant women,&rdquo; explains Denise Jamieson, M.D., MPH, a medical officer with the <a href="http://www.cdc.gov/" target="_blank">Centers for Disease Control and Prevention</a> (CDC). Pregnant women comprise 1 percent of the population, yet 6 percent of deaths from H1N1 have been among pregnant women.</p>
<p><strong>What should a pregnant woman do if she thinks she may have the virus?</strong></p>
<p>Regardless of whether a woman has been vaccinated, she should call her doctor immediately if she experiences any symptoms, which include fever, cough, sore throat, runny or stuffy nose, body aches, headache, chills and fatigue (the same symptoms as the seasonal flu). This is also the case if she&rsquo;s been exposed to anyone confirmed to have the virus. Additionally, all people who contract the disease&mdash;especially pregnant women&mdash;are urged to receive antiviral drugs within 48 hours of the onset of symptoms. &ldquo;But even after 48 hours, there&rsquo;s still a benefit,&rdquo; Jamieson says.</p>
<p><strong>How do we know the vaccine is safe?</strong></p>
<p>Because the H1N1 vaccine is so similar to the seasonal flu vaccine, Fauci, who oversaw the government-sponsored trials, is confident about its safety. &ldquo;Very similar vaccines made by the same companies, with similar properties, have been safely given to pregnant women to prevent seasonal flu for decades,&rdquo; he says. &ldquo;These vaccines have a very good safety record, with no red flags.&rdquo;</p>
<p>Fauci adds that while there is a theoretical risk from any vaccine, the dangers of not getting vaccinated far exceed any potential ones the new vaccine may pose. Side effects most commonly associated with the seasonal flu vaccine include swelling and redness at the injection site.</p>
<p><strong>Will there be enough vaccine available for everyone who wants it?</strong></p>
<p>Because there was uncertainty about whether enough would be available, the CDC targeted five groups who have priority: pregnant women (they are at the head of the line); people who live with or care for children younger than 6 months of age; health care personnel with direct patient contact; children ages 6 months through 4 years; and those ages 5 through 18 years with chronic medical conditions.</p>
<p><strong>Does the H1N1 vaccine take the place of the seasonal flu vaccine?</strong></p>
<p>No. The vaccines protect against completely different viruses, and pregnant women are advised to get both.</p>
<p><strong>Does either vaccine contain thimerosal?</strong></p>
<p>According to the CDC, pregnant women may receive flu vaccines with or without thimerosal. While there&rsquo;s no evidence that the preservative is harmful to a pregnant woman or a fetus, some moms-to-be are concerned about exposure during pregnancy. As a result, thimerosal-free seasonal and 2009 H1N1 influenza vaccines are available in single-dose units for pregnant women and small children.</p>
